3 column "Holy Grail" layout

After having found the "Holy Grail" 3 column CSS developed by Matthew James Taylor quite useful, I had further requirements to customise the width of each column.

I decided to analyse his CSS and re-develop it with functionality to specify the column widths and their padding.
